Susan Gold, Certified Hypnotherapist Contact Information: Phone—(650) 532-0509, Email—susan@susangold.net, Education and Training: I was trained in hypnotherapy at the ISHI School of Hypnosis Training, which is located in Berkeley, CA. I am a Certified Member of the National Guild of Hypnotists, and I do annual continuing education to maintain my training at a high level. The National Guild of Hypnotists is the oldest and largest hypnotism organization in the world and its certification is the most widely recognized credential for the professional practice of the hypnotic arts. In addition to being a certified hypnotherapist, I am also certified by the State of California as a Domestic Violence Counselor. My highest degree is an M.A. in Creative Writing from San Diego State University. Notice: AS THE STATE OF CALIFORNIA HAS NOT ADOPTED EDUCATIONAL AND TRAINING STANDARDS FOR THE PRACTICE OF HYPNOTISM, THIS STATEMENT OF CREDENTIALS IS FOR INFORMATIONAL PURPOSES ONLY. Hypnotism is a self-regulating profession and its practitioners are not licensed by state governments. I am not a physician nor a licensed health care provider and may not provide a medical diagnosis nor recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis or any other type of treatment from a different practitioner, the client may seek such services at any time. In the event my services are terminated by a client, the client has a right to coordinated transfer of services to another practitioner. A client has a right to refuse hypnotism services at any time. A client has a right to be free of physical, verbal or sexual abuse. A client has right to know the expected duration of sessions, and may assert any right without retaliation. Redress: I am a certified member of the National Guild of Hypnotists, and practice in accordance with its Code of Ethics and Standards. If you have a complaint about my services or behavior that I cannot resolve for you personally, you may contact the National Guild of Hypnotists at P. O. Box 308, Merrimack, NH 03054-0308, (603) 429-9438, to seek redress. Other services than my own may be available to you in the community. You may locate such providers in the telephone book. Fees: A single session is $110.00. A three-session package is $300.00, and a six-session package is $570.00. Packages must be paid for at the time of the first session. You will be given 30 days notice of any change in fees. I accept cash and checks made out to Susan Gold. Sessions: Each session is approximately 90 minutes, sometimes shorter or longer depending on the techniques that I feel will be most effective. Please allow up to two hours for the first session. The number of sessions will vary depending on the number of issues you would like to work on, your desired outcomes, determination, and readiness for change. 24-Hour Cancellation Notice Requirement: Life is unpredictable, and I respect that you may need to cancel or reschedule an appointment; however, I do require a minimum of 24-hours notice. If sufficient notice is not given, you will be expected to pay a fee of $50.00. Emergencies are an exception. No Money Back Guarantee: Because I do not have control over my clients’ level of motivation, which is essential for hypnotherapy to be successful, I cannot offer a money back guarantee. Insurance: I suggest that you think of my services as something that you will pay for personally. In general, insurance companies do not like to cover hypnotic services; however, with advance notices, I will issue a receipt so that you may bill your insurance. My Approach: My approach is personal and practical. We will discuss your reasons for seeking hypnotherapy, the change you desire and the history of the problem, and I will choose a therapeutic approach that will best help you achieve your goals.
